Care Management Network of Vermont- The Care Management Network of Vermont is a collaboration of the 11 not-for-profit, Medicare-certified Home Health Agencies and Visiting Nurse Associations.   Find out how the Care Management Network of Vermont can help you, your family, and your friends assess your needs, help you develop a plan to get the care you need, put services in place, and monitor your ongoing care and changing needs.

Champlain Valley Agency on Aging Database Resource Directory- Champlain Valley Agency on Aging, Inc. (CVAA) is a private, non-profit, United Way organization. seving seniors in Addison, Chittenden, Franklin and Grand Isle counties in northwestern Vermont. This designation means that CVAA carries out programs authorized by the Older Americans Act - federal legislation that calls for local organizations to take the lead in creating a coordinated system of services for older persons across the country. CVAA is one of five Area Agencies on Aging in Vermont. This link brings you to a searchable database for senior specifi programs in the identified northwestern counties.

Compassionate Friends- Search for local Vermont chapters that provide bereavement support to families who have lost a child.

State of Vermont, Assisted Living  Facilities- Department of Disabilities, Aging and Independent Living provides a listing of all Assisted Living facilities in the state of Vermont.  Assisted Living Facilities are state licensed residences that combine housing, health and supportive services to support resident independence and aging in place. 

State of Vermont Nursing Homes-  Department of Disabilities, Aging and Independent Living provides a listing of all Long Term Care facilities in the state of Vermont.  Nursing homes provide nursing care and related services for people who need nursing, medical, rehabilitation or other special services.  They are licensed by the states and may be certified to particiapte in the Medicaid and or Medicare programs.   

Strength for Caring- A Vermont website aimed at supporting caregivers.  Find Vermont State caregiving resources, Vermont healthcare resources, Vermont caregiver support groups, and local and State resources for those taking care of a loved one.

Veteran Affairs VA has many resources available for Veterans and family members. Use the search feature to locate facility-related information quickly and accurately.  

VT211- Use Vermont 2-1-1 or dial 2-1-1 for information about health and human service organizations in your community.  At Vermont 2-1-1, callers will speak with a real person every time.  Call Specialists will problem solve and refer callers from throughout Vermont to government programs, community-based organizations, support groups, and other local resources.  211 maintains an extensive database of a variety of nonprofit services and programs available to Vermonters.  Let us help you navigate through the VT211 database system to meet your Vermont hospice and palliative care needs.

Chronic Conditions Information Network of Vermont and New Hampshire is a not-for-profit organization providing a  free electronic network of information and resources on chronic conditions for people and their providers living in Vermont and New Hampshire.  CCI  is an independent, non profit organization, whose purpose is to assist those who live or care for those with chronic conditions so that they are able to obtain accurate and relevant information in a timely manner.

Vermont Assembly of Home Health Agencies- The Vermont Assembly of Home Health Agencies is the professional association of the 11 not-for-profit Home Health Agencies and Visiting Nurses Associations of Vermont.

Vermont Ethics Network-  The Vermont Ethics Network website contains information and resources on advance directives, health care decision making, and current topics related to health care ethics, end-of-life care, palliative care and pain management.
